Residence document lost or stolen

Has your residence document been lost or stolen? Read what you need to do. 

Which residence documents?

  • Electronic residence card
  • Paper residence documents
    • Orange card (‘attestation of registration’)
    • Annex 15 or annex 49
    • Annex 8ter
    • Annex 3 (‘declaration of arrival) or 3ter (‘notification of presence’)
    • Annex 33 or 3septies (for border and mobility student)

Step 1: File a report with the police and Docstop

Report the loss or theft of your residence document to the police. You will receive an 'annex 12' or a certificate of loss ('attest van verlies').
Is it a paper residence document? Only report this if the document was still valid at the time of loss or theft.

Step 2: Apply for a new residence document

What do you need to bring?

  • Annex 12 or certificate of loss ('attest van verlies')
  • National passport or identity card
  • Did the document contain a photo? Then bring 1 passport photo.

Recovered your electronic residence card?

Did you declare your electronic residence card lost or stolen? Then you may no longer use or reactivate it. Have the recovered card destroyed by the city.
